After filling out the form you may Print the Form to Mail it and/or keep for your records. To Print the Form Click on File (top Menu Bar) then click on Print.

After you submit your order, we will send you an invoice showing the exact cost. You can then send your photos and/or t-shirts with the required 50% deposit. Note: A 50% deposit is required on all Custom Order Quilts. The Balance is due before the finished quilt will ship back to you. Allow an additional 10 business days for checks to clear.
